How to Apply

Complete and submit the application form below.
By submitting, you certify that the information provided is true and that there is a genuine need for assistance. You also accept our terms and conditions and agree that we may reach out with follow-up questions.
Please also provide a short essay (1,000 words or fewer) describing what music or sports mean to the applicant. Email this to requests@givingdogs.com.
